A Travel Document is a certification or identifying document containing the description and other personal circumstances of its bearer, issued for direct one-way travel to the Philippines with limited validity. Generally, a travel document, in lieu of a passport, may be issued to: a) a Filipino citizen returning to the Philippines who for one reason or another has lost his/her passport or cannot be issued a regular passport; and, b) a Filipino citizen being sent back to the Philippines. The following are the requirements for the issuance of a Travel Document: a) Two (2) copies of the Travel Document application form duly accomplished; b) Three (3) color ID pictures (examples in figure 1); c) Affidavit of Loss duly notarized; d) Original copy and photocopy of Thai police report translated into English; e) Photocopy of lost passport, or in lieu thereof, any documentary proof of Philippine citizenship; and, f) Payment of processing fee (Subject to change). |
