III. Registration Of Birth, Death Or Marriage PDF Print E-mail

The birth or death of a Filipino citizen in Thailand, or the marriage contracted by a Filipino in Thailand must be reported at the earliest opportunity to the Philippine Embassy, Bangkok for the proper registration with the Office of the Civil Registrar General Philippines.

1. Requirements for Report of Birth

a. Personal appearance of at least one parent;

b. Report of Birth from duly accomplished;

c. Four (4) color ID pictures (examples in figure 1);

d. Birth Certificate of the child translated into English and duly authenticated by the Thai Ministry of Foreign Affairs;

e. Copy and Four (4) photocopies of the Marriage Contract ofthe parents duly authenticated:

(e.1) If the marriage was solemnized in the Philippines, the marriage contract must be duly authenticated by the National Statistics Office ( Office of the Civil Registrar General), Office of the President, Malaca?ang (Legal Office), and by the Department of Foreign Affairs (Authentication Division).

(e.2) If the marriage was solemnized under the Thai Law, the marriage contract must be accompanied by an English translation duly authenticated by the Thai Ministry of Foreign Affairs.

f. Valid passports of the child's parents and four (4) photocopies of each passport.

2. Requirements for Report of Marriage

a. Marriage Contract translated into English and duly authenticatedby the Thai Ministry of Foreign Affairs;

b. Original Passport and photocopy of the passport's personal data and visa pages; and,

c. Six (6) copies of Report of Marriage from duly accomplished.

3. Requirements for Report of Death

a. Death Certificate issued by the appropriate Thai government agency translated by the Thai Ministry of Foreign Affairs;

b. Four (4) photocopies of Death Certificate;

c. Valid passport of the deceased; and,

d. Report of Death form duly accomplished.
