VII. Travel Of Unaccompanied Children PDF Print E-mail

The travel of Filipino children below 18 years of age outside fo the Philippines, unaccompanied either by his or her parents or legal guardian, requires a Travel Clearance Certificate from the Department of Social Welfare and Development (DSWD) in Manila. The following documents are required to be submitted to the DSWD for the issuance of the said Certificate:

a) Certified copy of the minor's Birth Certificate;

b) Marriage Certificate of minor's parents, if applicable;

c) Affidavit of Consent from both parents duly notarized;

d) Affidavit of Support of sponsor in country of destination , duly notarized;

e) Latest Income Tax Retrun/Information of parents;

f) Three (3) color ID pictures of minor (exmaples in Figure 1);

g) Photocopy of passport and visa of travelling companion, if any.
